Job summary

Suvida Healthcare is seeking a Care Coordinator to support our Chronic Care and Transition of Care programs. Working with the Nurse Care Manager, you will conduct daily patient follow-ups, process DME and home health orders, triage calls, and retrieve hospital records for recently admitted patients.

You will collaborate with physicians and the care team to ensure high-quality, patient-centered care for high-risk patients, identify barriers to care, and help guide patients through the continuum

Qualifications

• Minimum 3 years of experience as a Medical Assistant or LPN/LVN.
• Certification as Medical Assistant required (AAMA/CCMA).
• Completion of MA program required; LPN/LVN license preferred.
• English/Spanish bilingual preferred.

Responsibilities

• Conduct daily patient follow-ups for high-risk patients in the Chronic Care Program.
• Process DME and home health orders accurately.
• Receive inbound triage calls and coordinate appropriate responses to patient needs.
• Retrieve hospital records for recently admitted patients.
• Collaborate with Nurse Care Manager to ensure seamless patient care delivery.
• Support transitions of care from acute to home or other facilities.
