Job summary

CSG is seeking a Case Manager to provide casework services for individuals and families in the Children’s Blended Case Management program. This role involves interviewing clients, developing service plans, coordinating resources, and maintaining detailed records.

Expect on-call coverage, travel to community sites, and collaboration with schools, physicians, and social services to support child welfare goals.

Qualifications

• Bachelor’s degree or RN/High school diploma with social science credits and 2 years in public/private human services
• Ability to work flexible hours
• Strong interview and assessment skills
• Valid driver's license and access to reliable vehicle

Responsibilities

• Interviews individuals and family representatives to gather psychosocial information
• Completes assessments and creates goal-oriented service plans
• Coordinates services for individuals and families
• Maintains records and prepares reports
• Participates in trainings and uses resources for client needs
• Makes referrals to community services
• Maintains communication with families and service providers
• Travels to locations for community/home-based services
• Provides on-call coverage and emergency response

Education

Bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, criminal justice, nursing, education

RN or high school diploma with social services experience
