Job summary

EMPACT-SPC is seeking a full-time Children's Case Manager in the City of Maricopa. The role involves crisis intervention, support, transportation and education to families and children, with coordination across departments and partners.

The candidate will complete family needs assessments, make referrals, and ensure continuity of care while maintaining professionalism and flexibility to meet client needs.

Qualifications

• Bachelor's degree or behavioral health experience preferred.
• Course of Study in psychology, counseling, social work, or related field.
• Must have fingerprint clearance or be eligible to obtain one.
• Valid AZ driver's license with clean record in last 3 years.
• Bilingual Spanish preferred.
• Must be at least 21 years old.

Responsibilities

• Provide direct crisis intervention, support, transportation and education to families and children.
• Complete family needs assessments and make referrals to partners.
• Ensure continuity of care and coordinate communications with departments or collaborators.
• Flex scheduling to meet client and family needs.

Education

Bachelor's Degree in Behavioral Health related field

Course of Study: Psychology, counseling, and social work

Must have Fingerprint clearance card or be eligible to obtain one

Must have a valid AZ driver's license with no major infractions in past 3 years

Bi-lingual (Spanish) is preferred

Must be at least 21 years of age at time of employment
