PATH People Acting To Help, Inc. in Philadelphia, PA is seeking a Children’s Case Manager to support youth and families affected by serious mental illness or emotional disorders.

The role focuses on strengths-based assessments, service planning, CASSP principles, and coordination of psychiatric services to keep youth in the least restrictive setting. A Bachelor's in Human Services and 1–2 years in behavioral health are required.

Qualifications

• Bachelor’s Degree in Human Services field from an accredited college/university.
• One to two years experience in the behavioral health field.
• Experience working with children and adolescents preferred.
• Use of own personal vehicle and valid driver’s license required.

Responsabilités

• Provide a comprehensive strengths based assessment for each assigned child/youth
• Develop with each child/youth and family, a unified service plan addressing the needs for treatment, skill development, social and emotional development and social service needs
• Ensure that the principles developed by Pennsylvania Child and Adolescent Service System Program (CASSP) are followed in providing services for youth and their families
• Coordinate psychiatric services as indicated
