Job summary

Waypoint's Western Region is seeking a skilled Care Coordinator to join our Children’s Behavioral Health Home program. You will work with families and caregivers to assess needs, develop an individualized plan of care, and organize and monitor services.

The role requires strong communication, reliability, and the ability to navigate multiple technology systems. A BA/BS in human services or related field (or 1 year relevant experience with another degree) is required, with driving to client sites

Qualifications

• BA/BS in human services or related field required, or BA/BS in unrelated field with at least 1 year of relevant children's services experience.
• Knowledge of the Maine system of care preferred.
• Reliable vehicle with proper insurance and a valid Maine driver’s license.

Responsibilities

• Assess and identify needs of children birth to 21 with developmental disabilities or behavioral health needs.
• Develop individualized care plans and coordinate services with families/caregivers.
• Monitor progress and adjust supports in collaboration with the care management team.

Our team is growing! Waypoint's Western Region (formerly known as The Progress Center) is seeking a skilled, motivated, and highly organized professional to join our growing Children's Behavioral Health Home program.

About Waypoint:

Waypoint offers comprehensive services, including a special purpose private school for children with autism and other developmental disabilities, case management and outpatient therapy services for adults and children, and residential and in-home supports for adults.

About the Role:

The Care Coordinator supports children and adolescents from birth to 21 years of age who have developmental disabilities and/or behavioral health needs by working with the child's family/caregivers to assess and identify needs, develop an individualized plan of care, and organize and monitor services and supports. The Care Coordinator is a member of the Children's Care Management Team that also includes Nursing, Psychiatry, Medical, and Family Supports providers.

Qualifications:

• BA/BS in human services or closely related field or
• BA/BS in unrelated field + minimum 1 year experience in relevant children's services
• Knowledge of the system of care in Maine preferred

Requirements:

• Reliable vehicle with proper insurance
• Valid driver's license issued by state of residence
• Strong interpersonal communication skills, proficiency with written, spoken English, and ability to utilize various technology systems for record keeping/plan monitoring

Job Type: Full-time
