Job summary

Waypoint is expanding its Adult Case Management team in western Maine, based in Oxford with a hybrid remote work option after onboarding. The Case Manager supports adults with intellectual disabilities and their families, coordinating care, advocacy, and service monitoring.

Responsibilities include planning care, initiating referrals, and collaborating with a multi-disciplinary team to ensure health, safety, and satisfaction with services. A sign-on bonus is offered.

Qualifications

• BA/BS in human services or related field, or BA/BS in unrelated field + minimum 1 year experience in adult IDD services.
• Must have reliable vehicle with proper insurance and a valid Maine driver’s license.
• Proficiency in speaking, reading, and writing English with strong interpersonal and communication skills.

Responsibilities

• Identify needs and develop a plan of care for adults with IDD.
• Initiate referrals and monitor services with a multi-disciplinary team.
• Advocate for services and support licensure efforts for staff.

Our department is growing! Enjoy a collaborative team-based approach with small caseloads? Waypoint is seeking a skilled, motivated, and highly organized professional to join our growing Adult Case Management team to provide services throughout the Western Maine area. Case Managers provide support to adults with intellectual disabilities/autism and their families; serving as an advocate, ensuring choice and satisfaction with services, and monitoring the individual's health and safety. Our home office for Adult Case Management is located in our Oxford, Maine location; this is a hybrid remote position after successful onboarding.

About Waypoint:

Waypoint offers comprehensive services, including a special purpose private school for children with autism and other developmental disabilities, case management and outpatient therapy services for adults and children, and residential and in-home supports for adults.

About the Role:

The Case Manager assists the individual and their team to identify needs and develop a plan of care, initiate referrals and monitor services, while working closely with the multi-disciplinary team. The Case Manager coordinates and advocates for services identified in the plan, monitors progress and evaluates the effectiveness of services. Supervision and support to achieve and maintain social work licensure (LSW) is also available.

Qualifications:

• BA/BS in human services or closely related field or
• BA/BS in unrelated field + minimum 1 year experience in adult IDD services
• Knowledge of the system of care in Maine preferred

Requirements:

• Reliable vehicle with proper insurance
• Valid driver's license issued by state of residence with clean driving record
• Must be proficient in speaking, reading, and writing English with strong interpersonal and communication skills
• Must be able to complete agency and state mandated training programs with proficiency
