Job summary

Waypoint’s Western Region is seeking a skilled Care Coordinator to join our Children’s Behavioral Health Home program, serving birth to 21-year-olds with developmental disabilities or behavioral health needs. You will assess needs, craft individualized care plans, and organize services with families and the care team.

Ideal candidates bring strong interpersonal skills, English proficiency, and reliability with record-keeping systems.

Qualifications

• BA/BS in human services or related field, or BA/BS in unrelated field with at least 1 year experience in children's services.
• Knowledge of the Maine system of care preferred.
• Reliable vehicle with insurance; valid driver's license required.

Responsibilities

• Assess and identify needs for children birth to 21 with developmental disabilities or behavioral health needs.
• Develop an individualized plan of care and coordinate services for the child and family.
• Monitor progress and collaborate with the Children’s Care Management Team (Nursing, Psychiatry, Medical, Family Supports).

About Waypoint:

Waypoint offers comprehensive services, including a special purpose private school for children with autism and other developmental disabilities, case management and outpatient therapy services for adults and children, and residential and in-home supports for adults.

About the Role:

The Care Coordinator supports children and adolescents from birth to 21 years of age who have developmental disabilities and/or behavioral health needs by working with the child's family/caregivers to assess and identify needs, develop an individualized plan of care, and organize and monitor services and supports. The Care Coordinator is a member of the Children’s Care Management Team that also includes Nursing, Psychiatry, Medical, and Family Supports providers. Qualifications:

• BA/BS in human services or closely related field or
• BA/BS in unrelated field + minimum 1 year experience in relevant children’s services
• Knowledge of the system of care in Maine preferred

• Strong interpersonal communication skills, proficiency with written, spoken English, and ability to utilize various technology systems for record keeping/plan monitoring.
