Job summary

Waypoint Maine, Inc. is seeking a Care Coordinator to support children and adolescents from birth to 21 with developmental disabilities and/or behavioral health needs.

You will work with families to assess needs, develop an individualized plan of care, and organize services through the Children's Care Management Team. The role requires BA/BS in a related field or equivalent experience, reliable transport, and strong communication skills in English.

Qualifications

• BA/BS in human services or closely related field or BA/BS in unrelated field + minimum 1 year experience in relevant children's services.
• Knowledge of the system of care in Maine preferred.
• Reliable vehicle with proper insurance and valid state's driver's license.

Responsibilities

• Assess and identify needs, develop an individualized plan of care, and organize and monitor services and supports for children and adolescents birth to 21.
• Collaborate with the child's family/caregivers and the Children's Care Management Team.
• Utilize technology systems for record keeping and plan monitoring.

Our team is growing! Waypoint's Western Region (formerly known as The Progress Center) is seeking a skilled, motivated, and highly organized professional to join our growing Children's Behavioral Health Home program.

About Waypoint

Waypoint offers comprehensive services, including a special purpose private school for children with autism and other developmental disabilities, case management and outpatient therapy services for adults and children, and residential and in-home supports for adults.

About the Role

The Care Coordinator supports children and adolescents from birth to 21 years of age who have developmental disabilities and/or behavioral health needs by working with the child's family/caregivers to assess and identify needs, develop an individualized plan of care, and organize and monitor services and supports. The Care Coordinator is a member of the Children's Care Management Team that also includes Nursing, Psychiatry, Medical, and Family Supports providers.

Qualifications

• BA/BS in human services or closely related field or
• BA/BS in unrelated field + minimum 1 year experience in relevant children's services
• Knowledge of the system of care in Maine preferred

• Valid driver's license issued by state of residence
• Strong interpersonal communication skills, proficiency with written, spoken English, and ability to utilize various technology systems for record keeping/plan monitoring.
