Job summary

Pyramid Consulting, Inc. is seeking a Child Welfare Case Manager to work in Orlando, FL on a 6+ month contract with hybrid work. You will manage a 100-member caseload, coordinate care, and document in electronic health records while ensuring compliance with policies.

The ideal candidate has 2+ years in child welfare, experience connecting families to local resources, and proficiency with Microsoft Office; foster care/adoption and health plan experience are a plus.

Qualifications

• 2+ years of experience in child welfare.
• Experience connecting families/children to local resources for services.
• Proficiency in Microsoft Office Suite (Outlook, Word, Excel).
• Nice to have health plan experience; foster care and/or adoption experience.

Responsibilities

• Shift: 8am - 5pm, Monday through Friday; Weekend work may be needed during emergency situations.
• Provide care management to assist with coordination of care, including assessing health care needs and referring to health plan providers.
• Educate members on benefits and expanded benefits while offering community resources to address social determinants of health needs.
• Manage a caseload of 100 members, documenting in an electronic health record, completing outreach notes, assessments, and care plans.
• Ensure compliance with care management policies/procedures, including outreach and documentation expectations, with monthly audits to verify chart compliance.
• Participate in a comprehensive onboarding process, including training on tools and processes over a 4-week period.

Immediate need for a talented Child Welfare Case Manager. This is a 06+months contract opportunity with long-term potential and is located in Orlando, FL (Hybrid).
