The role supports social workers in guiding families after initial assessments, helping them access services and work toward safer, more stable homes.

The ideal candidate will coordinate resources, monitor progress, advocate for families, and help with reunification where possible.

Qualifikationen

• Bachelor-level or associate degree in a relevant field preferred or equivalent experience.
• Strong written and verbal communication skills are essential.
• Ability to coordinate services and resources for families.
• Proven ability to work independently and in teams.

• Coordinate services and resources tailored to each family’s needs.
• Monitor progress and celebrate milestones.
• Support permanency planning to ensure stable, loving homes for children.
• Advocate for families through collaboration with service providers and legal partners.
• Help address underlying issues and promote reunification where possible.
• Assist with facilitating interactions between children and parents.
• Supervise interactions as directed by the case manager.
• Transport families to appointments as needed.
• Organize and maintain confidential files.
