Job summary

The Care Navigator Case Manager for Adult Services at Health Care and Rehabilitation Services in Springfield, VT, offers a full-time opportunity with a salary range of $44,850 to $58,909 annually plus a $500 sign-on bonus.

You will lead service planning and care coordination for children and families receiving mental health services, manage referrals, provide primary care screening and monitoring, and collaborate with community partners to improve outcomes.

Qualifications

• Associate’s Degree and two years experience and/or Bachelor’s Degree.
• Ability to establish supportive relationships with children, youth and families.
• Knowledge of holistic and therapeutic case management services.
• Experience in the mental health field.
