Qualifications

  • Strong communication, organization, documentation, and interpersonal skills.
  • Ability to collaborate with families, providers, and community partners.

  • Facilitate Birth to Three team and IFSP meetings.
  • Assist families in identifying strengths, needs, goals, and services.
  • Develop and coordinate IFSPs, including six-month and annual reviews.
  • Coordinate evaluations, assessments, medical services, and other supports.
  • Monitor children’s progress and the delivery of services.
  • Connect families with service providers, advocacy resources, and community programs.
  • Maintain accurate and timely case documentation.
  • Assist families with transition planning as children exit Birth to Three.
  • Collaborate with families, providers, school districts, and community partners.
  • Maintain compliance with Birth to Three, Part C, HFS 90, and program requirements.
  • Participate in required meetings, trainings, and professional development.
