CarePath Behavioral Health in Alabama seeks a Child and Adolescent Care Coordinator (case manager) to provide mental health case management for children and teens within our program.
The role involves completing needs assessments, developing case plans, coordinating with families, schools, and community services, and helping to stabilize clients in the community while coordinating with In Home and Outpatient teams. Prior bachelor’s degree and experience with youth are required or preferred.

Preferred Experience: Previous work with children and adolescents
Duties and Responsibilities:Provide mental health case management services to children and adolescents.Complete needs assessments and develop caseplans. Works closely with the family unit and community programs for the welfare of the child/adolescent.Works to keepindividuals stabilized in the community settings and assists in securing psychiatric hospital placement as needed. Works closely with the C&A In Home team to provide needed case management services as well as functions as the liasion between In Home and Ouptatient services.Assist with crisis situations as needed.
