Duties and Responsibilities: Provide mental health case management services to children and adolescents. Complete needs assessments and develop case plans. Works closely with the family unit and community programs for the welfare of the child/adolescent. Works to keep individuals stabilized in the community settings and assists in securing psychiatric hospital placement as needed. Works closely with the C&A In Home team to provide needed case management services as well as functions as the liasion between In Home and Ouptatient services. Assist with crisis situations as needed.
