Responsibilities

Provide enterprise-wide executive leadership for nursing and social work case management and related services.
Develop and execute a unified, patient-centered case-management strategy.
Mentor high-performing interdisciplinary teams and manage staffing models.
Improve length of stay, readmissions, and coordination across care settings.
Present performance results and strategic recommendations to the Board and executive leadership.
