Responsibilities

• Provide enterprise-wide executive leadership for case management, social work, utilization management, and discharge planning.
• Develop unified strategies aligned with clinical, operational, financial, and health-equity priorities.
• Lead patient progression, transitions, and continuity of care across the care continuum.
• Oversee utilization management, payer interactions, and optimization of resources.
• Present performance metrics and strategic recommendations to executive leadership and the Board.
