Job summary

Regency Integrated Health Services is seeking an RN Charge Nurse for the Bastrop, TX area to support a 6pm–6am shift at Lost Pines Nursing and Rehabilitation Center. You will lead nursing rounds, assess residents, and coordinate care with physicians and staff to ensure high-quality outcomes.

Qualifications

• Has completed an RN program at an accredited school.
• Active Registered Nurse license in Texas in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Ensure care plans are implemented as directed.
• Administer medications as prescribed.
• Conduct rounds with physicians and document visits.
• Perform narcotic reconciliation with shifts.
• Identify changes in resident condition and act per policy.
• Oversee and supervise aides.
• Complete incident and accident reports as required.
• Discharge residents per facility policy.
• Communicate with resident contacts about status changes.
