Job summary

Regency Integrated Health Services at Bastrop Lost Pines Nursing and Rehabilitation Center in Bastrop, TX seeks a licensed LVN Charge Nurse for a full-time role on the 6am-6pm shift. The LVN will support quality resident care and coordinate with physicians and staff to ensure care plans are carried out during rounds.

Requirements include completion of an LVN program and Texas licensure. Experience in skilled nursing is preferred, with responsibilities including medication administration, rounds

Qualifications

• Has completed an LVN program at an accredited vocational school or college.

Responsibilities

• Observe, assess, document, and report resident condition/changes during rounds.
• Assure implementation of care plans as directed.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes in the condition of residents and take necessary action per facility policies and procedures.
• Oversees and monitors aides.
• Initiate and complete Incident and Accident Reports per facility policy.
• Discharge residents per facility policy.
• Communicate with resident’s point of contact when they ask for a status update or if there is a change in condition.
• Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator
