Job summary

Windsor Nursing and Rehabilitation Center of Bastrop seeks an RN Charge Nurse to ensure high-quality care for residents during the 10pm–6am shift. The role emphasizes assessment, medication administration, and collaboration with physicians and the care team.

Responsibilities include rounds, documenting changes, implementing care plans, and maintaining accurate records in the clinical chart. Active RN license in Texas required; skilled nursing facility experience preferred.

Qualifications

• RN program completed at an accredited institution.
• Texas RN license in good standing.
• Experience in skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition/changes during rounds.
• Ensure care plans are implemented as directed.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with shifts and identify significant changes in resident condition.
• Oversee and monitor aides, initiate and complete incident/accident reports.
• Discharge residents per facility policy.
• Complete Nursing Assessments for Physician Calls before contacting the physician of any change in condition.
