Job summary

Bastrop Lost Pines Nursing & Rehabilitation Center seeks an RN Charge Nurse to ensure quality care and residents' well-being. You will observe, assess, and document changes, administer medications, and coordinate physician rounds and nurse oversight to maintain high standards of care.

The role requires an active Texas RN license, graduation from an accredited RN program, and prior SNF experience is preferred.

Qualifications

• Has completed an RN program at an accredited school.
• Active RN license in Texas in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes in resident condition and take action per policies.
• Oversee and monitor nursing aides.
• Initiate and complete Incident/Accident Reports per policy.
• Discharge residents per policy.
• Complete Nursing Assessments prior to physician calls for changes in condition.
• Communicate with residents' contacts regarding status updates.
• Support other projects as assigned by Director of Nurses or Administrator.
