Creative Solutions In HealthCare is seeking a Charge Nurse - LVN or RN to lead care teams and ensure high-quality resident care. The role requires reliable attendance, multitasking, and collaboration with staff to implement care plans and administer medications.

Ideal candidates hold an RN or LVN license in good standing, demonstrate strong clinical assessment, documentation, and communication skills, and respond effectively in emergencies.

Qualifications

• Current RN or LVN license in good standing.
• Strong clinical assessment and documentation skills.
• Ability to follow care plans and physician orders.
• Effective communication and teamwork.

Responsibilities

• Direct Care Teams: guide Nurse Aides to ensure appropriate resident care.
• Administer Medications safely as prescribed.
• Implement Care Plans in accordance with policies and orders.
• Perform Treatments to maintain resident health and comfort.
• Transcribe physician orders accurately and promptly.
• Document Resident Care in charts and complete nursing assessments.
• Collaborate during Shift Reports for care continuity.
• Educate Support Staff on nursing policies and procedures.
• Maintain Safety Standards through infection control and body mechanics.
• Address Staffing Needs by arranging replacement staff as necessary.
