Creative Solutions In HealthCare is seeking a Charge Nurse - LVN or RN to lead care teams and ensure high-quality resident services. The role emphasizes dependable attendance, multitasking, and collaboration with nursing staff to support patient well-being.

Responsibilities include directing care teams, administering medications, implementing care plans, performing treatments, documenting care, and maintaining safety standards in a compliant facility.

Qualifications

• Licensed RN or LVN in good standing.
• Strong clinical assessment and documentation skills.
• Excellent communication and teamwork abilities.
• Ability to organize workflows and multitask in a dynamic setting.
• Responsive to emergencies and detail-oriented.

Responsibilities

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
