Job summary

Creative Solutions in Healthcare, Inc. is seeking a Charge Nurse - LVN or RN to join our Winnie, Texas team. You will lead care teams and ensure residents receive safe, compassionate care in a dynamic setting.

Ideal candidates hold RN or LVN licensure, demonstrate strong assessment and documentation skills, and communicate effectively. You will coordinate care plans, administer medications, transcribe orders, and participate in shift reports to maintain continuity of care.

Qualifications

• Must hold RN or LVN licensure in good standing.
• Strong clinical assessment and documentation abilities.
• Excellent communication and workflow organization.
• Ability to respond efficiently to emergencies with attention to detail.

Responsibilities

• Direct Care Teams: Provide guidance to Nurse Aides to ensure appropriate care for residents.
• Administer Medications: Properly and safely administer resident medications as prescribed.
• Implement Care Plans: Execute resident care plans in compliance with company policies, federal guidelines, and physician orders.
• Perform Treatments: Deliver treatments as required to ensure resident health and comfort.
• Transcribe Orders: Accurately and promptly transcribe physician orders.
• Document Resident Care: Maintain timely and accurate documentation in resident charts and complete required nursing assessments.
• Collaborate During Shift Reports: Participate in shift reports to ensure seamless communication and care continuity.
• Educate Support Staff: Implement nursing policies and procedures, educating staff on the care needs of residents.
• Maintain Safety Standards: Promote a safe working environment by complying with infection control and body mechanic procedures.
• Address Staffing Needs: Respond to staffing shortages by arranging for replacement staff as necessary.
