Signature HealthCARE is seeking a registered nurse for Rockford Rehab & Wellness Center in Louisville, KY. This role offers opportunities to provide direct nursing care, manage ADLs, supervise nursing assistants, and contribute to care plans within a resident-centered culture.

Ideal candidates have an active RN license, CPR/BLS, at least one year of clinical experience (long-term care preferred), experience in supervisory roles, and familiarity with EHR systems.

Qualifikationen

• Active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.

Aufgaben

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
