Job summary

Signature HealthCARE at Heritage Hall Rehab & Wellness Center in Lawrenceburg, KY is seeking an experienced Registered Nurse to provide direct nursing care, supervise nursing staff, and ensure high-quality resident care in a long-term care setting.

The role requires an active RN license in Kentucky, strong clinical skills, and knowledge of LTC regulations. EHR proficiency and teamwork with interdisciplinary staff are essential, with competitive pay and comprehensive benefits offered.

Qualifications

Current, active RN license in the state of practice.
CPR/BLS certification preferred (American Heart Association or American Red Cross).
Minimum one year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.
Effective verbal and written communication skills.
Ability to work independently, prioritize tasks, and exercise sound clinical judgement.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
Develop, implement, and review individualized resident care plans with the interdisciplinary team.
Complete accurate and timely clinical documentation per facility policy and procedure.
Coordinate resident admissions, transfers, and discharges per established procedures.
Participate in quality assurance activities, staff education, and regulatory surveys as needed.
Perform other duties as assigned by the Director of Nursing or designee.
