Signature HealthCARE is seeking an experienced RN in Kentucky to provide direct nursing care, supervise nursing staff, and coordinate resident care plans. You will ensure patient safety and accurate documentation, while adhering to regulatory requirements.

The role emphasizes ADL support, medication administration, and collaboration with interdisciplinary teams to maintain high-quality care across our facilities.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during shifts.
• Monitor and observe residents’ status; notify physician of changes.
• Develop and review individualized resident care plans with the team.
• Complete accurate clinical documentation per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
