Job summary

Signature HealthCARE at Colonial Rehab & Wellness Center in Bardstown, KY seeks an experienced RN to join a resident-centered team delivering high-quality care across short-term and long-term services.

You will supervise nursing assistants, administer medications, assist with ADLs, and develop resident care plans in collaboration with the interdisciplinary team; strong documentation and EHR skills are required.

Qualifications

• Active RN license in Kentucky
• BLS/CPR certification preferred
• Minimum 1 year clinical nursing experience; long-term care experience preferred
• Experience in supervisory roles preferred
• Strong observation, medication administration, and documentation skills
• Familiarity with electronic health records (EHR) systems

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
