Job summary

Priority Management Group, LLC is seeking a licensed practical nurse (LPN/LVN) to join our caring nursing team at Brookhaven Nursing and Rehabilitation Center in Carrollton, TX. This open-shift role supports 6p-6a and requires collaboration with the Medical Director, physicians, and DON to deliver high-quality care.

You will assess residents, document findings, and support clinical staff while helping residents grow in their care plans.

Qualifications

• Must have a current LPN/LVN license in good standing with the State Board of Nursing.
• Excellent patient care skills and compassion for residents.
• Ability to work well in a team and communicate effectively with staff and physicians.

Responsibilities

• Conferring with the Medical Director and attending physician regarding residents assigned to you.
• Consulting with the physician concerning resident evaluation and assist the Director of Nursing in planning nursing services.
• Helping team members grow their careers and providing quality care to residents.
• Examining residents and their records to identify early signs of health issues.
• Performing administrative duties such as completing forms, reports and charting.

What You’ll be Doing

• Conferring with the Medical Director and the attending physician regarding specific residents assigned to you.
• Consulting with the physician concerning resident evaluation and assist the Director of Nursing in planning and developing the nursing services to be performed for the resident.

• Examining residents and their records and charts to distinguish between normal and abnormal findings in order to recognize early stages of serious physical, emotional or mental problems.
• Performing administrative duties regarding assigned residents, such as completing medical forms, reports, evaluations, studies, charting, etc.
