Job summary

Care Initiatives is seeking a Charge Nurse - Licensed Practical Nurse (LPN) in Iowa to provide comprehensive nursing care and leadership within a skilled nursing community. You will assess residents, deliver direct care, administer medications, and lead a team to ensure high-quality, compliant care.

The role emphasizes compassionate, patient-centered care, mentoring CNAs, and collaboration with physicians and families to support health goals in a resident-focused setting.

Qualifications

• Valid Licensed Practical Nurse (LPN) license in good standing (Iowa).
• Current CPR Certification.
• Documented completion of State-approved LPN Supervisory Course or ability to obtain within first 90 days.
• Knowledge of federal and state long-term care regulations and laws.
• Strong empathy, respect, patience, and discretion in care.

Responsibilities

• Conduct ongoing assessments of residents' clinical status and contribute to individualized care plans with the interdisciplinary team.
• Deliver and oversee direct nursing care including wound treatments and IV therapy as permitted by scope.
• Administer medications and prescribed treatments with safe, accurate documentation.
• Serve as a clinical leader on the unit by coordinating resident care and communicating with physicians, leadership, residents, and families.
• Supervise, mentor, and support CNAs and other staff, promoting accountability and high-quality care.
• Ensure adherence to state/federal regulations and infection prevention practices; maintain a safe, compliant environment.
• Provide education and emotional support to residents and families about care plans and health goals.
