Full-time Charge Nurse / (LPN)
This is a full-time hourly position. The expected workschedule is primarily Monday–Friday daytime hours with rotating/on-callresponsibilities. Additional hours may occasionally be required based onresident needs, staffing, emergencies, or facility operations.
Position Summary
The Charge Nurse / Assistant Director of Nursing (LPN) is a hands-on nursing leadership position responsible for providing quality residentcare while assisting the Director of Nursing with the daily coordination and oversight of nursing services.
This position serves as a working leader on the nursingteam, providing direct resident care while also assisting with staffsupervision, clinical coordination, documentation, communication, and otherroutine nursing-management functions. The Charge Nurse will help ensure thatresidents receive safe, compassionate, individualized care and that nursingpractices are consistent with facility policies, applicable regulations, andestablished standards of care.
This position is intended to function as a working AssistantDirector of Nursing, with limited administrative and managementresponsibilities while maintaining regular involvement in resident care. Thisposition is expected to lead by example, maintaining a professional,respectful, and resident-centered approach. The individual in this position should be comfortable balancing hands-on nursing care with leadership responsibilitiesand should serve as a dependable resource for the nursing team.
The goal of this position is to provide the Director ofNursing with reliable day-to-day clinical support while maintaining a strongpresence at the bedside and ensuring residents receive safe, compassionate, andconsistent care.
Job Responsibilities
- Provide direct nursing care to residents inaccordance with their care plans, physician orders, and facility policies.
- Perform nursing assessments and monitorresidents for changes in condition.
- Administer medications and treatments as orderedand within the scope of the LPN license.
- Monitor vital signs, symptoms, behaviors,wounds, nutrition, hydration, and other clinical needs.
- Communicate significant changes in residentcondition to the Director of Nursing, physician, responsible party, or otherappropriate healthcare professionals.
- Assist with coordination of physician visits,laboratory services, pharmacy needs, therapy services, and outside medicalappointments.
- Respond appropriately to resident emergenciesand initiate appropriate interventions.
- Promote resident dignity, privacy, independence,and quality of life.
- Serve as a daily clinical resource and leaderfor nursing and caregiving staff.
- Provide guidance and direction to CNAs,medication aides, and other nursing personnel as appropriate.
- Assist with assigning daily duties and ensuringadequate coverage of resident care needs.
- Monitor staff performance and adherence toestablished nursing procedures.
- Provide coaching, education, and feedback tostaff regarding resident care and clinical practices.
- Assist the Director of Nursing with orientationand training of new nursing employees.
- Help identify staffing concerns and communicatescheduling or performance issues to the Director of Nursing.
- Assist the Director of Nursing with theday-to-day operation of the nursing department.
- Help maintain accurate and timely clinicaldocumentation and resident records.
- Assist with reviewing medication administrationrecords, treatment records, care plans, progress notes, and other requireddocumentation.
- Participate in care plan meetings and helpensure resident care plans are implemented appropriately.
- Assist with incident/accident reporting,investigation, and follow-up.
- Help monitor infection prevention and controlpractices.
- Assist with quality assurance and performanceimprovement activities.
- Assist with ensuring compliance with facilitypolicies, state regulations, and applicable licensing requirements.
- Communicate resident, staffing, and clinicalconcerns to the Director of Nursing and Executive Director as appropriate.
- Serve as a leadership resource for the nursingdepartment in the absence of the Director of Nursing, within the scope ofassigned authority.
- Maintain effective communication with residents,families, physicians, pharmacies, home health agencies, hospice providers, andother healthcare professionals.
- Communicate resident changes, concerns, andneeds to appropriate members of the care team.
- Promote teamwork and professionalism amongnursing and caregiving staff.
- Participate in staff meetings, resident careconferences, and other meetings as assigned.
- Maintain confidentiality of resident andfacility information.
- Participate in the facility's nursing on-callrotation or designated on-call responsibilities.
- Provide telephone support and clinical guidanceto staff when on call.
- Respond appropriately to urgent resident-careconcerns and determine when escalation to the Director of Nursing, physician,emergency services, or other appropriate personnel is necessary.
- Assist with urgent staffing or clinical issuesas reasonably required during on-call periods.
Job Qualifications
- Current, unrestricted Licensed Practical Nurse(LPN/LVN) license in the applicable state.
- Previous experience in assisted living,long-term care, memory care, skilled nursing, or a similar healthcare settingpreferred.
- Previous charge nurse, lead nurse, supervisory,or nursing leadership experience preferred.
- Current CPR/BLS certification or ability toobtain certification within the required timeframe.
- Knowledge of medication administration, residentassessments, care planning, infection prevention, and documentationrequirements.
- Strong communication, organization, andleadership skills.
- Ability to work effectively with residents,families, healthcare professionals, and nursing staff.
- Ability to prioritize multiple responsibilitieswhile maintaining quality resident care.
- Demonstrated professionalism, dependability, andsound clinical judgment.
- Ability to stand and walk for extended periods.
- Ability to assist with resident transfers,positioning, and mobility as required by the resident's care needs and facilitypolicies.
- Ability to lift, push, pull, bend, kneel, andreach as required to perform nursing duties.
- Ability to respond promptly to residentemergencies.
- Ability to perform required documentation andcomputer-based tasks.
