Focused Post Acute Care Partners seeks a Charge Nurse for Focused Care at Linden, Linden, TX. The role requires an active LVN or RN license and CPR certification. You will lead nursing staff, ensure resident safety, and implement physician orders with a focus on quality care.

You will document assessments, develop care plans, and communicate with families and the health team. The position is on-site with benefits and opportunities for growth within FPACP.

Qualifications

• Must hold a current LVN or RN license.
• CPR certification required.
• Ability to understand, remember and carry out English instructions.
• Ability to complete assignments in a timely manner.
• Flexible and positive under intermittent interruptions.

Responsibilities

• Provide nursing care as prescribed by physicians and nursing standards.
• Lead licensed and non-professional staff delivering resident care.
• Plan, organize, and direct nursing functions for residents.
• Assign tasks appropriate to staff skill levels.
• Maintain professional patient-care standards and confidentiality.
• Document nursing history and assess patient condition.
• Develop care plans and communicate changes to the team.
• Adhere to infection-control policies and procedures.
• Respond effectively in urgent or emergency situations.
• Coordinate with multidisciplinary team and families.
