Job summary

Focused Post Acute Care Partners is seeking a Charge Nurse to join Focused Care at Hamilton, Hamilton, TX. You will provide nursing care as prescribed by the physician and supervise staff to ensure high-quality resident care in a fast-paced facility.

Must hold a current LVN/RN license with CPR certification and have good English communication skills. The role emphasizes leadership, adherence to infection-control policies, and maintaining patient confidentiality.

Qualifications

• Must have a current LVN/RN license.
• CPR certification required.
• Ability to understand, remember and carry out instructions in English.
• Ability to complete assignments in a timely manner.

Responsibilities

• Lead or direct licensed and non-professional staff in delivering direct resident care.
• Ensure resident safety and maintain standards of patient care.
• Develop care plans and assign tasks according to staff skill level.
• Document nursing history and monitor patient condition.
• Communicate with patients, families, and health team regarding care plans.
