Focused Post Acute Care Partners in Baytown, TX, seeks a Charge Nurse to lead nursing care for residents and supervise licensed and support staff. You will ensure safety, implement physician orders, and maintain high standards of practice.

The role requires current LVN/RN license, CPR certification, and strong communication. Candidates should be able to document care, collaborate with the health team, and respond decisively to emergencies.

Qualifications

• Current LVN/RN license.
• CPR certification required.
• Ability to understand and follow verbal or written instructions in English.
• Ability to complete assignments in a timely manner.
• Flexible and positive with intermittent interruptions.

Responsibilities

• Provide nursing care and direct nursing functions for patients in the unit.
• Ensure resident safety and maintain care standards.
• Identify problems and guide personnel to solutions.
• Create a climate that promotes growth and job satisfaction for staff.
• Implement physicians' orders accurately and promptly.
• Enforce infection-control policies and procedures.
• Maintain confidentiality and professional conduct.
• Coordinate with nursing staff, medical staff, other departments and visitors.
• Obtain and document comprehensive nursing histories.
• Perform basic patient assessments and document findings.
• Record vital signs and note changes for further investigation.
• Notify physicians of changes in condition.
• Develop care plans before end of duty and ensure continuity for incoming staff.
• Follow established procedures to provide safe care.
• Take accountability for clinical care and supervise non-professional staff.
• Communicate with patients, families, and health care team to achieve care goals.
• Perform essential technical skills relevant to unit care.
• Respond effectively in emergencies.
• Adhere to policies and dress code.
• Utilize electronic medical record charting (point‑click care).
