Responsibilities

• Coordinates the provision of spiritual care for patients, families and staff.
• Develops and evaluates the spiritual plan of care through assessments.
• Educates hospital staff in the provision of spiritual care.
• Coordinates with a multi-disciplinary team to facilitate holistic care, including ethics.
• Documents patient care concerns and interventions and participates in committees.
• Contributes expertise in cultural/faith diversity, advance care planning, end-of-life decisions and grief processes.
• Works in a multi-staff, culturally diverse setting.
