Job summary

Pinnacle Services Incorporated in Minnesota seeks a CFSS Consultation Coordinator to support the MHCP provider serving CFSS. The role includes educating individuals, helping to write and review service delivery plans, and guiding on covered services while collaborating with lead agencies.

You will work with a diverse caseload in the metro area, balance independent tasks with teamwork, and after training the position moves to fully remote work with an emphasis on quality and compliance.

Qualifications

• All employees working directly with persons served must meet the education requirements listed or the education substitution described below.
• Have a bachelor’s degree or higher in one of the following fields: Occupational therapist, Occupational therapy assistant, Physical therapist, Physical therapy assistant, Psychologist, Social worker, Speech-language pathologist or audiologist, Professional recreation staff, Professional dietitian, Psychology, Sociology, Counseling, Special education, Rehabilitation counseling, Other human services fields.

Responsibilities

• Educate the person served about CFSS.
• Educate the person served about the agency model and budget model.
• Help the person served write their CFSS service delivery plan, to the extent the person served desires.
• Review the CFSS service delivery plan and submit it to the lead agency for approval.
• Offer guidance to the person served on whether the CFSS service delivery plan is complete and only contains covered services.
• Provide the person served with a list of CFSS provider agencies or FMS providers.
• Respond to questions from the person served throughout the year.
• Help the person served change their service delivery plan, model and/or providers, if applicable.
• Complete a semi-annual review if necessary.
• Help DHS with surveys and data collection, at DHS’ request.
• Document complaints and provide them to DHS upon request.
• Review their complaint policy annually.
• Have policies and procedures to meet the needs of culturally diverse people receiving services.
• Share information from DHS with the people served when requested.
