Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, Healthcare, Rehabilitation, or related field.
• One year of full-time experience providing direct services to individuals with disabilities or older adults.
• Personal experience coordinating your own long-term support services.

Responsibilities

• Develop and manage a service delivery plan with participants based on county assessment under CFSS.
• Coordinate with agencies, lead providers, and DHS to fulfill service plans.
• Maintain ongoing communication with participants and service providers to monitor plan success.
