Qualifications

• BS/BA degree or acceptable experience in Human Services or related field or a minimum of one year’s experience in Human Services or related field.
• Valid driver’s license.
• Reliable transportation with at least liability insurance.

Responsibilities

• Serve as liaison between individuals and the Provider Led Arkansas Shared Savings Entity (PASSE) to ensure services meet goals identified in the Person Centered Service Plan (PCSP).
• Implement PCSP plans for individuals on assigned caseload.
• Ensure budget information is completed and submitted timely.
• Ensure individuals attend all medical appointments and obtain required paperwork.
• Make monthly contact with each consumer on assigned caseload.
• Complete the home requirement checklist as needed.
• Ensure staffing needs are met and that the plan of care is fully utilized.
• Perform other duties as assigned.
