AltaPointe Health seeks a Peer Specialist to promote self-determination and recovery, assisting individuals in identifying strengths and wellness goals. You will participate in treatment teams, support goal planning, and help clients access community services and benefits.

You will model self-advocacy, teach wellness strategies, and connect clients to support groups while educating staff on consumer-centered care and crisis planning.

Qualificações

• High School diploma or equivalent; Peer Specialist Certification through State of Alabama Department of Mental Health.
• Must be 23 years of age and hold a valid Alabama driver license with acceptable driving record.
• Maintain proof of current liability insurance; proficient in basic computer word-processing.

Responsabilidades

• Promoting self-determination, recovery, self-advocacy, and self-direction; assisting individuals in identifying strengths and wellness goals.
• Attend treatment team meetings to promote self-directed advocacy and assist in goal planning and seeking community services.
• Assist individuals in obtaining information about benefits (SSI, SSDI, Medicaid, etc.); engage in outpatient programs to encourage recovery.
• Model self-advocacy skills and teach wellness management strategies; help develop personal self-management plans/tools; utilize approaches like IMR/WMR, WRAP.
• Connect individuals to community support groups and educate staff to integrate consumer preferences into services.
