Qualifications

• 1 year directly related experience.
• CPR for Healthcare/BLS or equivalent certification is required; state certification may be preferred.

Responsibilities

• PATIENT SAFETY - Follow patient safety-related policies, procedures and protocols.
• PATIENT SAFETY - Demonstrate proactive approach to patient safety by questioning current policies and processes.
• PATIENT SAFETY - Identify and report environmental conditions that may put a patient at risk.
• PATIENT SAFETY - Report potential patient safety concerns and near misses in a timely manner.
• PATIENT CENTERED MED - Adhere to the Patient Centered Medical Home or Patient Centered Specialty Practice expectations.
• NAVIGATION/PATIENT ADVOCACY - Help patients access community resources and financial assistance.
