Job summary

Community Bridges, Inc. (CBI) is seeking a Certified Peer Specialist with lived recovery experience to support individuals in crisis and transition planning. The role emphasizes reducing emotional impact and enhancing engagement with treatment services.

The position requires coordinating care with the treatment team, families, and social supports to ensure smooth discharge planning and ongoing recovery in the DC area.

Qualifications

• Highschool diploma or GED required.
• Associate Degree or higher preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders required.
• Health care related experience preferred.

Responsibilities

• Provide lived experience to minimize emotional impact of treatment and increase recovery likelihood.
• Assist in transition to appropriate level of care with the treatment team.
• Ensure coordination of care with all parties involved in the patient’s case.
• Involve family and social supports identified in the treatment plan in discharge planning.
