Qualifications

Responsibilities

• Follows the therapist's plan of care and documents communications with the evaluating therapist.
• Implements patient-centered, occupation-based interventions and adapts to patient needs.
• Collaborates with patients, families and care team throughout the rehabilitation process.
• Educates patients, families, caregivers and other health care providers using effective methods.
• Keeps clear, objective documentation to support care decisions and payer requirements.
• Participates in quality assurance activities to improve service delivery.
• Communicates treatment progress with the medical team and supports discharge planning.
