Treatment and evaluation materials provided
Onsite Director of Rehab support
Deliver care and therapeutic exercises based on treatment plans developed by the OT
Assist residents with activities that promote daily living skills, independence, and quality of life
Accurately document therapy sessions and monitor resident progress
Collaborate with therapists, nurses, residents, and family members to support individualized care goals
Educate residents, families, and staff on home exercise and restorative programs
Participate in interdisciplinary team meetings and quality improvement initiatives
Provide coverage at nearby facilities when needed based on patient and staffing needs
Graduate of an accredited Occupational Therapy Assistant program
Current state license/registration as a COTA
Skilled nursing or long-term care experience is a plus
Ability to safely assist residents with mobility and therapeutic activities
Ability to lift up to 50 lbs
Strong communication, organization, and time-management skills
Compassionate, dependable, and resident-focused approach
