Mill Creek Center for Nursing and Rehabilitation seeks a compassionate Occupational Therapist (OT) to join our team in providing high-quality resident care. The OT will perform evaluations, develop personalized treatment plans, deliver therapy, and supervise support staff in a collaborative, patient-centered environment.

Qualifications

• Valid state licensure as an Occupational Therapist.
• Experience in long-term care highly valued.
• Strong communication and organizational skills.
• Passion for working with geriatric population.

Responsibilities

• Perform patient evaluations and develop individualized treatment plans based on physician orders.
• Deliver occupational therapy treatments and adjust plans as needed to achieve functional goals.
• Supervise COTAs, rehab techs, and therapy students as required.
• Maintain accurate and timely documentation in accordance with regulatory standards.
• Collaborate with interdisciplinary team members, including nurses, physicians, and physical therapists.
• Educate residents, families, and staff on therapeutic techniques and home exercise programs.
• Participate in discharge planning and facility quality improvement initiatives.

Responsibilities
