Responsibilities

• Provide individualized therapy interventions to help residents improve independence and functional abilities.
• Assist with therapeutic activities to enhance mobility and activities of daily living (ADLs).
• Collaborate with an interdisciplinary team to implement and adjust care plans.
• Accurately document treatments, progress, and outcomes in accordance with regulatory guidelines.
• Deliver care with professionalism, compassion, and attention to detail.

Las Colinas Post Acute is a premier skilled nursing and rehabilitation facility serving San Bernardino County. We provide comprehensive short-term and long-term care, rehabilitation, and respite services in a safe and welcoming setting.
