Job summary

Las Colinas Post Acute is seeking a Registered Physical Therapist (PT) to join our rehabilitation team at our Ontario, CA facility. This is an excellent opportunity to provide patient-centered care while collaborating with a supportive clinical team.

Responsibilities include evaluating residents, designing individualized plans, and delivering interventions to improve mobility, strength and balance. Documentation and teamwork are essential in this role.

Qualifications

• Active California Physical Therapist license
• Graduate of an accredited Physical Therapy program
• Strong commitment to patient-centered care
• Excellent communication and teamwork skills
• Experience in a skilled nursing or post-acute setting preferred

• Evaluate residents and develop individualized treatment plans
• Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
• Collaborate with an interdisciplinary team to implement and adjust care plans
• Supervise and support therapy assistants as appropriate
