Job summary

Putnam Nursing & Rehabilitation Center is seeking a Full-Time Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. The COTA will work with Occupational Therapists and the interdisciplinary team to help residents improve ADLs, mobility, and independence.

Requirements include a current NYS COTA license and AOTCB certification. Experience in SNF or rehab settings is preferred; knowledge of MDS/PRI is a plus.

Qualifications

• Current COTA license in good standing with the NYS Board of Education.
• Certification through AOTCB required.
• Experience in skilled nursing facility or rehabilitation setting preferred.
• Knowledge of MDS, PRI, and care planning preferred.

Responsibilities

• Assist the Occupational Therapist in implementing occupational therapy treatment programs.
• Provide therapy services to improve ADLs, mobility, and functional independence.
• Document therapy sessions and resident progress accurately.
• Participate in care plan meetings and interdisciplinary team meetings.
• Provide adaptive equipment and recommend environmental modifications when appropriate.
• Educate residents and caregivers on therapy techniques and treatment plans.
• Maintain therapy supplies, equipment, and treatment areas.
• Follow all facility policies and rehabilitation documentation standards.
