Putnam Nursing & Rehabilitation Center is actively hiring a full-time Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. The COTA will work closely with Occupational Therapists to enhance residents’ ADLs, mobility, and independence, while documenting progress and contributing to care plans.

The role requires a current NYS COTA license, AOTCB certification, and prior experience in a skilled nursing or rehab setting.

Formación

• Current COTA license in good standing with NYS Board of Education.
• Certification through AOTCB required.
• Experience in skilled nursing facility or rehab setting preferred.
• Knowledge of MDS, PRI, and care planning preferred.

Responsabilidades

• Assist OT in implementing therapy treatment programs.
• Provide therapy services to improve ADLs, mobility, and independence.
• Document therapy sessions and resident progress accurately.
• Participate in care plan meetings and interdisciplinary team meetings.
• Provide adaptive equipment and environmental modifications when appropriate.
• Educate residents and caregivers on therapy techniques and plans.
• Maintain therapy supplies, equipment, and treatment areas.
• Follow facility policies and rehabilitation documentation standards.

• Certified Occupational Therapy Assistant (COTA) provides adaptive equipment and recommend environmental modifications when appropriate.

• Follow all facility policies and rehabilitation documentation standards.

Requirements

• Current COTA license in good standing with the NYS Board of Education.

• Experience in skilled nursing facility or rehabilitation setting preferred.
• Knowledge of MDS, PRI, and care planning preferred.
