Job summary

Putnam Nursing & Rehabilitation Center is seeking a full-time Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. The role supports Occupational Therapists in improving residents' ADLs, mobility, and independence, with emphasis on compassionate, interdisciplinary care in a 160-bed facility.

You will document progress, participate in care planning, and help create adaptive environments.

Qualifications

• Current COTA license in good standing with the NYS Board of Education.
• Certification through AOTCB required.
• Experience in skilled nursing facility or rehabilitation setting preferred.
• Knowledge of MDS, PRI, and care planning preferred.

Responsibilities

• Assist OT in implementing therapy programs.
• Provide therapy services to improve ADLs, mobility, independence.
• Document therapy sessions and resident progress accurately.
• Participate in care plan meetings and interdisciplinary team meetings.
• Provide adaptive equipment and recommend environmental modifications.
• Educate residents and caregivers on therapy techniques.
• Maintain therapy supplies, equipment, and treatment areas.
• Follow facility policies and rehabilitation documentation standards.

Our Certified Occupational Therapy Assistant (COTA) will work closely with Occupational Therapists and the interdisciplinary care team to help residents improve activities of daily living (ADLs), strength, and independence.

• Certified Occupational Therapy Assistant (COTA) will assist the Occupational Therapist in implementing occupational therapy treatment programs.

• Certified Occupational Therapy Assistant (COTA) documents therapy sessions and resident progress accurately.

• Certified Occupational Therapy Assistant (COTA) provides adaptive equipment and recommend environmental modifications when appropriate.

• Follow all facility policies and rehabilitation documentation standards.
