Job summary

Putnam Nursing & Rehabilitation Center is a 160-bed skilled nursing facility seeking a Full-Time Certified Occupational Therapy Assistant (COTA) to join our growing rehabilitation team. You will work closely with Occupational Therapists and the care team to help residents improve ADLs, strength, and independence.

Qualifications

• Current COTA license in good standing with the NYS Board of Education.
• Certification through AOTCB required.
• Experience in skilled nursing facility or rehabilitation setting preferred.
• Knowledge of MDS, PRI, and care planning preferred.
• Covid-19 vaccinations are encouraged but NOT MANDATED; vaccination offered per CDC guidance.

Responsibilities

• Assist the Occupational Therapist in implementing occupational therapy treatment programs.
• Provide therapy services to improve ADLs, mobility, and functional independence.
• Document therapy sessions and resident progress accurately.
• Participate in care plan meetings and interdisciplinary team meetings.
• Provide adaptive equipment and recommend environmental modifications when appropriate.
• Educate residents and caregivers on therapy techniques and treatment plans.
• Maintain therapy supplies, equipment, and treatment areas.
• Follow all facility policies and rehabilitation documentation standards.
