Certified Occupational Therapy Assistant

We are seeking a Certified Occupational Therapist Assistant. This role involves a combination of center-based work (70%) and travel for home visits (30%). The specialist will be responsible for managing fall prevention strategies, implementing Durable Medical Equipment (DME), and providing caregiver education on safety and preventative care for falls in the home.

Key Responsibilities

• Conduct fall follow-ups and assessments in the center.
• Perform home visits to implement DME, install grab bars, and conduct fall follow-ups.
• Assist Occupational Therapists (OTs) with follow-ups and dynamic interventions.
• Evaluate new and suboptimal home setups to ensure safety and recommend first interventions such as DME or orientation to PACE.
• Provide caregiver education on safety and preventative care for falls in the home.
• Coordinate with transportation to facilitate home visits and ensure timely follow-ups.
• Gather information and prepare DME for home visits.

Qualifications

Must Haves:

• 1+ year working with the frail and elderly (geriatric)
• Proficiency in operating a computer and navigating systems.
• Current certification from the National Board for Certification of Occupational Therapy (NBCOT).

Plusses:
